We're seeking a skilled and empathetic therapist to provide vital mental health services to patients in our HIV/AIDS Services Program. You'll work closely with an interdisciplinary team to deliver holistic, high-quality care, making a real difference in the lives of individuals facing unique challenges.

What You'll Do:
Provide counseling sessions to assigned patients.
Complete all required documentation accurately and on time.
Perform initial assessments and ongoing care.
Participate in the development of individualized care plans.
Address co-occurring disorders.
Work as a valued member of the multidisciplinary treatment team.
Coordinate with case managers and patient support staff.
Connect patients to internal and external resources as needed.
Perform other duties as assigned.
What You'll Need:
Active registration in good standing as a Licensed Marriage and Family Therapist (LMFT) or Licensed Clinical Social Worker (LCSW).
Location: Hillcrest Family Health Center, San Diego, CA (hybrid work model).
